Important Health Coverage Tax Document: Form 1095‑C
In accordance with the Paperwork Reduction Act, the 2025 Form 1095‑C health coverage tax documents will be available upon request. All active employees may also access their tax forms through Employee Self‑Service. To request a copy of your 1095-C form, please email humanresources@northeast.edu, mail to Northeast Community College PO Box 469, Norfolk NE 68701; or call 402-844-7043.
